Promotion Details:
Earn 50 points on iD Lifestyle 4, MySV, Sensity Family, and EX4. Order all three receive an additional 150 points for a max payout of 300 points per order. Promotional points are not eligible for tier bonus. Points are earned for the month the customer enrolls. For Example: Customer enrolls on July 15 they earn points starting July 1. If a customer enrolls Aug 15, they earn points starting on Aug 1.
The Promotion is open only to professional eyecare practices that are not excluded, debarred, or suspended, nor owned or controlled, in whole or in part, by a person who is excluded, debarred, or suspended, from participation in any federal health care program as defined at 42 U.S.C. § 1320a-7b(b). Program Expires on September 30, 2024.
